Preventive Botox at 20 years:

Do you really need it?

Honestly? Maybe not.

I know it's not what you expect a doctor at an aesthetic clinic to tell you. But it's the truth, and it's the first thing I tell younger patients who come asking about it.
Today we live in an era where social networks have normalized starting aesthetic treatments earlier and earlier. And although prevention does have an important place in aesthetic medicine, I also firmly believe that not every patient needs to start at the age of 20.

Aesthetic medicine is not about doing more. It's about knowing when it really makes sense to do so.

The idea makes sense.

Every time you frown, squint, or raise your eyebrows, you're folding the skin in the same place. If you do it enough times over the years, those creases stop disappearing when you relax your face. They remain.

Botulinum toxin smooths that movement before wrinkles form. And a 2025 systematic review spanning 20 years of research confirms that it really does work as prevention, in the right patient and at the right dose. That last part matters more than people realize.

So... The concept of preventive Botox consists precisely of smoothing that movement before the lines are deeply marked.

The right patient, at the right time and with a natural application.

But is it for you?

If you're already noticing wrinkles at rest (on your forehead, between your eyebrows, around your eyes) or if your parents aged prematurely and you see that you're on the same path, it's worth talking about.

If you have smooth skin and you're here for something you saw on Instagram, go home, buy a good sunscreen, use retinoids and vitamin C; skincare is key! And he will be back in a few years.

Sunscreen is, without a doubt, the  most effective anti-aging  tool out there. I mean it. 

What I really do in consultation

If treatment is adequate, I don't start with botulinum toxin.

First I assess the actual quality of the skin:

  • hydration,

  • sun damage,

  • pigmentation,

  • collagen production,

  • texture,

  • elasticity.

Because many times the problem is not a wrinkle... but a skin that is dehydrated, inflamed or with early sun damage.

I start by improving the quality of the skin: 

Depending on the case, I often prefer to start with treatments that really improve skin health:

  • Radiofrequency to stimulate collagen

  • IPL or Intense Pulsed Light for Pigmentation and Sun Damage

  • Medical peels for texture and radiance

  • Bioregenerators such as Profhilo or Rejuran to restore deep hydration.

Botox works best on skin that is already healthy, it's a finishing touch, not a starting point.

Fillers at this age? Almost never.

The unnecessary use of fillers in young patients can disrupt the natural harmony of the face and lead to exaggerated results over time.
In my practice I do not seek to transform faces. I seek  to preserve freshness, structure and naturalness, something that I always explain to my patients is: the goal should never be to "freeze" the face.

The real luxury in aesthetics today is in seeing you:

  • rested

  • fresh

  • healthy

  • natural without losing your expression or your essence.
    When done well, the result should not be noticeable... only to perceive oneself.

The most important anti-aging tool is still sunscreen

Although many people are looking for the most innovative treatment, the reality is that the best preventative treatment is still one of the simplest: daily sunscreen.

Cumulative sun exposure is one of the main causes of: 

  • premature aging,

  • stains,

  • loss of collagen,

  • flaccidity,

  • fine lines.

That's why no aesthetic treatment replaces a good skincare routine and constant sun protection.

So... do you really need preventive Botox?

It depends on your skin, your genetics, your habits, and your goals.
